Documentation

Reports patients take home. Documentation payers expect.

Diagnostic PDFs, Letters of Medical Necessity, and clinical notes — produced from the same patient record, in the same iPad app. Documentation is a workflow output, not a side trip.

Diagnostic reports

Gait, posture, and ROM produce shareable PDFs.

Every diagnostic — gait, posture, range of motion — finishes as a PDF the patient can take home and the practitioner can attach to a Letter of Medical Necessity. Findings are risk-graded; raw measurements are alongside interpreted summaries; severity is colour-coded with the same chip system the app uses internally.

Letter of medical necessity

A payer-ready packet, assembled in one place.

The LMN generator assembles a cover letter, HCPCS codes, ICD-10 diagnoses, payer policy fields, and supporting documentation — clinical notes, 3D scans, diagnostic PDFs — into a single document attached to the device request. The structure follows the language payers expect; the practitioner customizes the narrative. The output is a PDF the practitioner submits; FitsPro doesn't promise approval.

Clinical notes

SOAP notes, with optional live transcription.

Add a clinical note from the patient record with subjective, objective, assessment, and plan sections. Toggle live transcription to capture the visit in real time — the practitioner reviews and edits before saving. Notes are linked to the encounter and surface in the patient summary used by the MCP connector.

Decision support, not a reimbursement guarantee. FitsPro produces clinical and payer-facing documentation. Coverage determinations are made by individual payers based on medical necessity criteria; FitsPro does not guarantee approval of any specific claim.
